Mood Check-In (PHQ-9)
A brief self-assessment to check how you've been feeling over the past two weeks.
About This Questionnaire
The PHQ-9 (Patient Health Questionnaire-9) is one of the most widely validated depression screening tools in the world. It asks about 9 common depression symptoms over the past two weeks, scored on a 0–3 scale.
What It Measures
- Depressed mood and anhedonia (loss of pleasure)
- Sleep and appetite disturbances
- Fatigue and concentration difficulties
- Feelings of worthlessness or guilt
- Psychomotor changes and thoughts of self-harm
Important Note
The PHQ-9 is a screening tool, not a diagnostic instrument. A high score suggests depression symptoms and is a reason to speak with a healthcare provider — it is not a diagnosis. Results are private and stored only on your device.
